Indications

- Fungal skin diseases, mycoses of skin folds, feet;
- Tinea versicolor, erythrasma, superficial candidiasis caused by dermatophytes, yeasts (including the genus Candida), mold, and other fungi and pathogens sensitive to clotrimazole;
- Mycoses complicated by secondary pyoderma.

Section: Method of application and doses

For external use. The cream is applied in a thin layer 2-3 times a day to previously cleaned (using soap with a neutral pH value) and dried affected areas of the skin and gently rubbed in.
The duration of treatment depends on the severity of the disease, the localization of pathological changes, and the effectiveness of therapy.
Treatment of dermatomycosis should be conducted for at least 4 weeks, and for tinea versicolor - 1-3 weeks. In cases of fungal diseases of the skin of the feet, it is recommended to continue therapy for at least 2 weeks after the symptoms of the disease have resolved.

Composition

Each gram of the preparation contains:

Active ingredient: clotrimazole 10.0 mg

Excipients: propylene glycol 50.0 mg, white petrolatum 120.0 mg, liquid paraffin 60.0 mg, cetomacrogol emulsifying wax 135.0 mg, benzyl alcohol 10.0 mg, methylparaben 1.5 mg, propylparaben 0.5 mg, butylhydroxytoluene 1.0 mg, sodium dihydrogen phosphate dihydrate 0.8 mg, sodium hydrogen phosphate 0.6 mg, purified water up to 1000 mg.

Homogeneous cream of white color.

• Hypersensitivity to clotrimazole or other components of the drug

Pregnancy and lactation:
Clinical and experimental studies have not established that the use of the drug during pregnancy or breastfeeding has a negative impact on the health of the woman or the fetus (child). However, the question of the appropriateness of prescribing the drug in the second or third trimester should be decided individually after consultation with a doctor. When prescribing the drug during the second or third trimesters of pregnancy, the balance between the expected benefits of the drug for the mother and the possible risks to the fetus should be considered. Application of the drug directly to the lactating breast is contraindicated.

Special instructions

It is not recommended to apply the medication to the skin in the eye area.
In patients with liver failure, liver function should be monitored periodically.
If signs of hypersensitivity or irritation occur, treatment should be discontinued.
If there is no effect within 4 weeks, the diagnosis should be confirmed.

Side effects

Allergic reactions in the form of itching, urticaria, burning, tingling at the sites of cream application. Erythema, blisters, swelling, irritation, and peeling of the skin may occur.

Overdose

The use of the cream in increased doses does not cause any reactions or conditions that are life-threatening.

Interaction

Amphotericin B, nystatin, and natamycin reduce the effectiveness of clotrimazole when used simultaneously. When using the cream, negative interactions with other agents are not known and should not be expected, as the absorption capacity of clotrimazole is very low.

Pharmacodynamics

Pharmacological action - a broad-spectrum antifungal agent for local use, antibacterial, antiprotozoal, trichomonacidal. The antimycotic effect of the active ingredient clotrimazole (an imidazole derivative) is associated with the disruption of ergosterol synthesis, which is a component of the cell membrane of fungi, altering membrane permeability and causing subsequent cell lysis. At fungicidal concentrations, it interacts with mitochondrial and peroxidase enzymes, resulting in an increase in hydrogen peroxide concentration to toxic levels, which also contributes to the destruction of fungal cells. It exhibits fungicidal and fungistatic activity against dermatophytes (Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, Microsporum canis), yeast-like and mold fungi (Candida spp., including Candida albicans, Torulopsis glabrata, the genus Rhodotorula, Pityrosporum orbiculare). It is active against the causative agent of tinea versicolor - Pityrosporum orbiculare (Malassezia furfur). Effective against gram-positive bacteria - the causative agent of erythrasma Corynebacterium minutissimum, as well as Staphylococcus spp., Streptococcus spp., and gram-negative bacteria - Bacteroides, Gardnerella vaginalis. At high concentrations, it is active against Trichomonas vaginalis.

Pharmacokinetics

Clotrimazole is poorly absorbed through the skin and mucous membranes and has virtually no systemic effect. The concentration in the deeper layers of the epidermis is higher than the minimum inhibitory concentration for dermatophytes. With external application, the concentration of clotrimazole in the epidermis is higher than in the dermis and subcutaneous tissue.
