Ferinekt – the preparation of iron. It represents karboksimaltozatny polynuclear complex of iron (III) hydroxide.
Following parenteral administration of macromolecular complex is captured by the reticulo-endothelial system and decomposes into iron and karboksimaltozu.
Iron into the bloodstream, where it binds to the transport protein transferrin.
Iron in complex with transferrin transferred to the cells of the body, where it is used for the synthesis of hemoglobin, myoglobin and large amounts of enzymes or accumulates as ferritin.
Active substance:
Ferric carboxymaltosate
Manufacturer
Vifor (International) Inc., Switzerland
Prescribing
Adults are prescribed by a doctor
Composition
1 ml. solution for intravenous administration contains:
active ingredient:
50 mg iron karboksimaltozat
Indications
anemia
Iron deficiency anemia is when oral iron preparations are ineffective or may not be used.
The diagnosis should be confirmed by laboratory tests.
Contraindications
- anemias not associated with iron deficiency;
- the symptoms of iron overload or disturbances of iron utilization;
- I trimester of pregnancy;
- Children under 14 years;
- hypersensitivity to the drug Ferinekt.
Precautions should be used when the drug hepatic insufficiency, acute or chronic infectious diseases (possibly inhibition of erythropoiesis), bronchial asthma, eczema, atopic allergy.
Side effects
Allergic reactions: often – rash; infrequently – urticaria, hypersensitivity reactions including anaphylactoid reactions.
From the nervous system: often – headache; dizziness; rarely – paresthesia.
Cardio-vascular system: Infrequent – lowering blood pressure, “tides” of blood to the skin.
The respiratory system: rarely – shortness of breath.
From the digestive system: often – nausea, stomach pain, diarrhea, constipation; infrequently – a change of taste, vomiting, dyspepsia, flatulence.
On the part of the bone-myshchenoy system: rarely – myalgia, back pain, arthralgia.
General reactions: rarely – fever, fatigue, chest pain, chills, malaise, peripheral edema.
Local reactions: often – reactions at the injection site.
From the laboratory parameters: often – increase in serum ALT, transient decrease in serum phosphate content of blood; rarely – increase of activity ACT, GGT, LDH.
How to accept, acceptance rate and dosage
Ferinekt drug to be administered intravenously (bolus or infusion) and by direct injection into the venous portion of the dialysis system.
Before opening the vials should be inspected for possible damage and sediment.
You can only use a homogeneous clear solution.
Drip: Ferinekt can be administered intravenously in a single dose to a maximum of 20 ml (1000 mg of iron), which should not exceed 0.3 ml (15 mg iron) / kg body weight or calculated cumulative dose.
It is impossible to assign a drip Ferinekt drug in a dose of 20 ml over 1 times a week.
Storage conditions
At a temperature of not higher than 30 ° C, in the original package (do not freeze)
Active substance
Ferric carboxymaltosate
Overdose
Symptoms: Ferinekt in excess of the needs of the body, can cause iron overload, manifested symptoms of hemosiderosis. Determination of iron metabolism as the serum ferritin and transferrin saturation percentage may aid in the diagnosis of excessive iron deposits in the body.
Treatment: symptomatic therapy. If necessary, it can be used chelated compound (e.g., deferoxamine).
