Indications

As part of the complex therapy for acute and recurrent forms of infectious diseases of the skin and mucous membranes of various locations (including genital), caused by the herpes simplex virus; viral diseases of the oral mucosa (aphthous stomatitis, lichen planus); shingles (Herpes zoster), chickenpox, Kaposi's varicelliform eczema, cytomegalovirus infection. How to use

Tablets are taken regardless of meals. Adults and children over 12 years old are prescribed 100-200 mg (1-2 tablets) 3-4 times a day; children aged 6-12 years - 100 mg (1 tablet) 2-3 times a day; children aged 3-6 years - 50-100 mg (½-1 tablet) 2-3 times a day.
The therapeutic effect of the drug is most pronounced when prescribed in the early stages of the disease. The duration of treatment depends on the form and severity of the disease:

• In cases of widespread rashes in acute and recurrent forms of simple herpes, both genital and extragenital localization, as well as in the presence of fever, lymphadenopathy, and other systemic phenomena, the drug is used for 5-14 days.

• In herpetiform eczema of Kaposi and cytomegalovirus infection - for 7-21 days as part of combination therapy.

• In aphthous stomatitis, the drug is prescribed for 5-15 days.

• In chickenpox (Varicella), shingles (herpes zoster) - for 5-21 days. In case of recurrences, repeated courses of treatment with the drug are conducted. To prevent recurrences of herpesvirus infections, the drug is prescribed one month after the end of treatment for 10-14 days.
The maximum daily dose of the drug for children aged 3 to 12 years is 300 mg (three tablets); for children over 12 years and adults - 800 mg (eight tablets).

Таблетки применяют независимо от приема пищи. Взрослым и детям старше 12 лет назначают по 100-200 мг (1-2 таблетки) 3-4 раза в сутки, детям 6-12 лет - по 100 мг (1 таблетка) 2-3 раза в сутки, детям 3-6 лет - по 50 - 100 мг (? -1 таблетки) 2-3 раза в сутки.
Терапевтический эффект препарата наиболее выражен при его назначении в начальном периоде заболевания. Длительность лечения зависит от формы и тяжести заболевания:

• В случае распространенных высыпаний при острых и рецидивирующих формах простого герпеса как генитальной, так и экстрагенитальной локализации, а также при наличии лихорадки, лимфоаденопатии и других общих явлениях препарат применяют в течение 5-14 дней.

• При герпетиформной экземе Капоши и цитомегаловирусной инфекции - в течение 7-21 дня в составе комплексной терапии.

• При афтозном стоматите препарат назначают в течение 5-15 дней.

• При ветряной оспе (Varicella), опоясывающем лишае (опоясывающем герпесе) (Herpes zoster)) - в течение 5-21 дня. При рецидивах проводят повторные курсы лечения препаратом. Для профилактики рецидивов герпесвирусных инфекций препарат назначают через месяц после окончания лечения в течение 10-14 дней.
Максимальная суточная доза препарата для детей от 3-х до 12-и лет составляет 300 мг (три таблетки); для детей старше 12-ти лет и взрослых - 800 мг (восемь таблеток)

Active ingredient:
Tetrahydroxyglucopyranosylxanthone (Alpizarin®) (calculated as 100% substance) - 0.1 g;
Excipients: lactose monohydrate, potato starch, calcium stearate.

Round, flat-cylindrical tablets with a score on one side and a bevel on both sides, from light yellow with a greenish tint or without tint to yellow with a greenish tint or without tint, with darker speckles.

• 1 tablet of the medication contains 0.1 g of milk sugar (lactose) and 0.0475 g of potato starch, which corresponds to 0.01 XE (bread units). The maximum daily dose of the medication for children aged 3 to 12 years contains 0.03 XE; for children over 12 years and adults - 0.08 XE.

Pharmacology

Alpizarin is derived from mangiferin, extracted from the leaves of the Indian mango - Mangifera indica L., belonging to the Anacardiaceae family.

Pharmacodynamics.
Alpizarin exhibits antiviral activity against DNA-containing viruses: herpes simplex virus types 1 and 2 (Herpes simplex), varicella-zoster virus (Varicella zoster), and herpes zoster (Herpes zoster), as well as cytomegaloviruses. In vitro experiments have demonstrated the inhibitory effect of tetrahydroxyglucopyranosylxanthone on the human immunodeficiency virus (HIV).
The inhibitory effect of the drug on viral reproduction is most pronounced in the early stages of its development. Alpizarin does not affect the activity of viral neuraminidase.
Alpizarin possesses immunostimulating properties regarding cellular and humoral immunity, with the ability to induce gamma-interferon production in blood cells.
Alpizarin inhibits the growth of gram-negative (Escherichia coli) and gram-positive (Staphylococcus aureus, Mycobacterium tuberculosis) bacteria, fungi (Microsporum canis), and pathogenic protozoa (Entamoeba histolytica, Trichomonas vaginalis). The mechanism of action of the drug is based on its ability to suppress bacterial nuclease activity.
Alpizarin has a moderate anti-inflammatory (anti-edematous) effect.

Tetrahydroxyglucopyranosylxanthene, when taken orally, is rapidly absorbed from the gastrointestinal tract. Unchanged tetrahydroxyglucopyranosylxanthene is detected in the blood plasma for 0.5-5 hours after administration.
The maximum concentration of tetrahydroxyglucopyranosylxanthene (4.5 µg/ml) is reached 1 hour after administration. Tetrahydroxyglucopyranosylxanthene is significantly bound to blood proteins (90-70%).
Extrarenal pathways predominantly account for the elimination of tetrahydroxyglucopyranosylxanthene. The excretion of unchanged tetrahydroxyglucopyranosylxanthene by the kidneys within 24 hours does not exceed 0.1%.

Фармакодинамика.
Алпизарин обладает противовирусной активностью в отношении ДНК-содержащих вирусов: вируса простого герпеса 1 и 2 типа (Herpes simplex), вируса ветряной оспы (Varicella zoster) и опоясывающего лишая (Herpes zoster), цитомегаловирусов. В экспериментах in vitro показан ингибирующий эффект тетрагидроксиглюкопиранозилксантена в отношении вируса иммунодефицита человека (ВИЧ).
Ингибирующее действие препарата на репродукцию вируса наиболее выражено на ранних этапах его развития. Алпизарин не влияет на активность вирусной нейраминидазы.
Алпизарин обладает иммуностимулирующими свойствами в отношении клеточного и гуморального иммунитета, способностью индуцировать продукцию гамма-интерферона в клетках крови.
Алпизарин ингибирует рост грамотрицательных (Escherichia coli) и грамположительных (Staphylococcus aureus, Mycobacterium tuberculosis) бактерий, грибов (Microsporum canis) и патогенных простейших (Entamoeba histolytica, Trichomonas vaginalis). В основе механизма действия препарата лежит способность подавления активности бактериальной нуклеазы.
Алпизарин обладает умеренным противовоспалительным (противоотечным) действием

Тетрагидроксиглюкопиранозилксантен при приеме препарата внутрь быстро всасывается из желудочно-кишечного тракта. В плазме крови неизмененный тетрагидроксиглюкопиранозилксантен определяется в течение 0,5-5 ч после применения.
Максимум концентрации тетрагидроксиглюкопиранозилксантена (4,5 мкг/мл) достигается через 1 час после его применения. Тетрагидроксиглюкопиранозилксантен в значительной степени связывается с белками крови (90-70 %).
Преобладают внепочечные пути выведения тетрагидроксиглюкопиранозил-ксантена. Экскреция неизмененного тетрагидроксиглюкопиранозилксантена почками за 24 ч не превышает 0,1 %
