Composition

bicalutamide 150 mg
Excipients: corn starch 107 mg, croscarmellose sodium 19.5 mg, lactose monohydrate 132.9 mg, povidone 7.6 mg, sodium stearyl fumarate 3 mg.
Coating composition: Opadry II white 12 mg.

Tablets coated with a film, white or almost white in color, round, biconvex.

Contraindications & warnings

Simultaneous use with terfenadine, astemizole, cisapride; pediatric age, increased sensitivity to bicalutamide.
Not prescribed to female patients.

Use with caution in patients with moderate to severe liver function impairment, as accumulation of bicalutamide in the body may occur.
During treatment, regular monitoring of liver function is required. If severe changes develop, bicalutamide should be discontinued.

Одновременный прием с терфенадином, астемизолом, цизапридом; детский возраст, повышенная чувствительность к бикалутамиду.
Не назначают пациентам женского пола.

С осторожностью применять у пациентов с умеренно выраженными и тяжелыми нарушениями функции печени, т.к. возможна кумуляция бикалутамида в организме.
В период лечения требуется регулярный контроль функции печени. В случае развития тяжелых изменений необходимо прекратить прием бикалутамида.

Side effects, overdose & interactions

Endocrine system: hot flashes, breast tenderness and gynecomastia, decreased libido.
Gastrointestinal system: abdominal pain, diarrhea, nausea, vomiting, elevated transaminase levels, jaundice, cholestasis.
CNS: possible asthenia, depression.
Dermatological reactions: alopecia, hair regrowth, itching, dry skin.
Other: hematuria.

When using bicalutamide for 28 days alongside midazolam, the AUC of midazolam increased by 80%.
Concurrent use of bicalutamide with cyclosporine or calcium channel blockers may potentiate or lead to adverse reactions.
Theoretically, there may be an increase in the concentration of bicalutamide in the blood plasma when used simultaneously with liver microsomal enzyme inhibitors, which may be accompanied by an increased frequency of adverse reactions.
In vitro studies have shown that bicalutamide may displace the anticoagulant warfarin from its plasma protein binding sites.

Со стороны эндокринной системы: приливы, болезненность молочных желез и гинекомастия, снижение либидо.
Со стороны пищеварительной системы: боли в животе, диарея, тошнота, рвота, повышение уровня трансаминаз, желтуха, холестаз.
Со стороны ЦНС: возможны астения, депрессия.
Дерматологические реакции: алопеция, восстановление роста волос, зуд, сухость кожи.
Прочие: гематурия.

При применении бикалутамида в течение 28 дней на фоне приема мидазолама, АUC мидазолама увеличивалась на 80%.
При одновременном применении бикалутамида с циклоспорином или блокаторами кальциевых каналов возможно потенцирование или развитие нежелательных реакций.
Теоретически возможно повышение концентрации бикалутамида в плазме крови при его одновременном применении с ингибиторами микросомальных ферментов печени, что может сопровождаться увеличением частоты возникновения нежелательных реакций.
Исследования in vitro показали, что бикалутамид может вытеснять антикоагулянт кумаринового ряда варфарин из участков связывания с белками плазмы.

Pharmacology

Anti-androgenic non-steroidal agent. By binding to receptors with affinity for androgens, it suppresses androgen activity, resulting in regression of prostate tumors. It does not possess other types of endocrine activity.

After oral administration, it is well absorbed from the gastrointestinal tract. Food intake does not affect bioavailability.
Binding to plasma proteins is 96%. It is extensively metabolized in the liver through oxidation and the formation of glucuronide conjugates.
It is excreted as metabolites in urine and bile in approximately equal proportions.
Accumulation of bicalutamide in the body is possible.

Антиандрогенное нестероидное средство. Связываясь с рецепторами, имеющими сродство к андрогенам, подавляет активность андрогенов, в результате этого наблюдается регрессия опухоли предстательной железы. Не обладает другими видами эндокринной активности.

После приема внутрь хорошо всасывается из ЖКТ. Прием пищи не влияет на биодоступность.
Связывание с белками плазмы составляет 96%. Интенсивно метаболизируется в печени путем окисления и образования глюкуронидных конъюгатов.
Выводится в виде метаболитов с мочой и желчью примерно в равных пропорциях.
Возможна кумуляция бикалутамида в организме.
