Heparin Ointment 25 g

The medication is indicated for use in adults and children over 1 year of age, and adolescents for the treatment of the following conditions:

• thrombophlebitis of superficial veins, superficial periphlebitis;

• subcutaneous hematoma;

• post-injection and post-infusion phlebitis;

• superficial mastitis;

• localized infiltrates and edema, injuries and bruises (including muscle tissue, tendons, joints);

• external hemorrhoids, inflammation of postpartum hemorrhoidal nodes;

• lymphedema (elephantiasis), lymphangitis.

For external use.
The ointment is applied in a thin layer to the affected area (at a rate of 0.5 – 1 g (2 – 4 cm of ointment) for an area with a diameter of 3 – 5 cm) and gently rubbed into the skin. The ointment is used 2 – 3 times a day daily until the inflammatory symptoms disappear, on average from 3 to 7 days.
The possibility of a longer treatment course is determined by the physician.
In cases of thrombosis of external hemorrhoidal nodes, rectal tampons are used; the ointment is applied to a cotton or linen pad, which is placed directly on the thrombosed nodes and secured. The ointment-soaked tampon is inserted into the rectum. The ointment should be used daily until symptoms disappear, on average 3 – 14 days.

Children over 1 year and adolescents
The dosing regimen is similar to that of adults.
Children under 1 year
The use of the medication in children under 1 year of age is contraindicated.

100 g of ointment contains:
active ingredients: sodium heparin - 10,000 IU (0.0833 g at sodium heparin activity of 120 IU/mg), benzocaine (anesthesin) - 4.0 g, benzyl nicotinate - 0.08 g,
excipients: glycerol (distilled glycerin) - 15.0 g, petroleum jelly - 6.0 g, stearin (stearic acid) - 5.0 g, corn oil - 5.0 g, emulsifier No. 1 - 8.0 g, methylparaben - 0.148 g, propylparaben - 0.048 g, purified water - up to 100.0 g.

Homogeneous ointment of white color with a yellowish tint.

• hypersensitivity to sodium heparin, benzocaine, benzyl nicotinate, or any of the excipients;

• children under 1 year of age;

• disruption of the integrity of the skin (open infected wounds, ulcerative-necrotic processes in particular).

With caution
Disorders of the blood coagulation system, thrombocytopenia.

With prolonged use of Heparin Ointment over extensive areas and simultaneous intake of oral medications affecting the coagulation system and/or hematopoiesis, it is necessary to monitor coagulation time and prothrombin time.
In cases requiring a long treatment course and with extensive areas of damage in pregnant women and women over 65 years of age, similar monitoring is recommended.
Do not apply to open wounds or in the presence of purulent processes.
The use of the ointment is not recommended in cases of deep venous thrombosis.
The product contains methylparaben and propylparaben, which may cause allergic reactions (including delayed reactions).

With prolonged use of the drug, local reactions may occur in the form of skin hyperemia, decreased sensitivity at the application site, allergic reactions (skin rash, itching).

Since the drug primarily has a local effect at therapeutic doses, cases of overdose are unlikely. With prolonged use and on large surfaces, hemorrhagic complications (heparin) may occur. In these cases, the use of the drug should be discontinued and symptomatic therapy should be initiated.
The symptoms of overdose of the components when used locally have not been studied.

The ointment is not prescribed for local use simultaneously with non-steroidal anti-inflammatory drugs (NSAIDs), tetracyclines, or H1-histamine receptor blockers.

Combined preparation for external use, the action of which is determined by the properties of its components. Direct-acting anticoagulant. Gradually released from the ointment, sodium heparin reduces the inflammatory process and has an antithrombotic effect. It promotes the resorption of existing thrombi and prevents the formation of new ones. It blocks thrombin synthesis and reduces platelet aggregation. It inhibits hyaluronidase activity and activates the fibrinolytic properties of blood.

Benzyl nicotinate has a vasodilatory and spasmolytic effect. It directly reduces the contractile activity of smooth muscles in blood vessels and internal organs. Benzyl nicotinate, by dilating superficial blood vessels, facilitates the absorption of heparin.

The local anesthetic benzocaine (anesthesin), by reducing the permeability of the cell membrane to sodium ions, blocks the conduction of nerve impulses. It prevents the occurrence of pain impulses at the endings of sensory nerves and their conduction along nerve fibers, reducing the intensity of pain sensations. When applied to the skin, it has a local analgesic effect.

When applied locally, only a small amount of sodium heparin is absorbed into the systemic circulation, which is primarily excreted by the kidneys. The pharmacokinetics of the active substances - benzyl nicotinate and benzocaine (anesthesin) - have not been studied with local application.

The ointment stimulates blood circulation and improves microcirculation, which contributes to the rapid recovery of tissues. It contains heparin, which is a direct anticoagulant, allowing for the prevention of thrombus formation. Heparin ointment can be used for the treatment of both adults and children.
