Indications
The drug Afalaza® is indicated for use in adults.
Benign prostatic hyperplasia.
Dysuric disorders (as part of combination therapy): frequent urges to urinate, difficulty urinating, pain and discomfort in the perineal area associated with benign prostatic hyperplasia.

Доброкачественная гиперплазия предстательной железы.
Дизурические расстройства (в составе комплексной терапии): частые позывы на мочеиспускание, затруднение при мочеиспускании, боль и дискомфорт в области промежности, сопровождающие доброкачественную гиперплазию предстательной железы.
How to use
Orally. 2 tablets, 2 times a day, in the morning and evening (keep in the mouth until completely dissolved - outside of meals).
Risca is not intended for splitting the tablet into parts.
In cases of dysuric disorders, the medication is recommended to be taken for 16 weeks.
At the doctor's recommendation, in cases of severe pain syndrome and dysuric disorders, taking the medication up to 4 times a day is indicated during the first 2-3 weeks of therapy.
To prevent prostate growth in patients with BPH and to reduce the risk of disease progression, the medication is recommended to be taken 2 times a day, in the morning and evening, for 12 months.

Внутрь. По 2 таблетки, 2 раза в сутки, утром и вечером (держать во рту до полного растворения - вне приема пищи).
Риска не предназначена для деления таблетки на части.
При дизурических расстройствах препарат рекомендуется принимать в течение 16 недель.
По рекомендации врача при выраженных болевом синдроме и дизурических нарушениях в первые 2-3 недели терапии показан прием препарата до 4 раз в сутки.
Для предотвращения роста предстательной железы у пациентов с ДГПЖ и снижения риска прогрессии заболевания препарат рекомендуется принимать 2 раза в сутки, утром и вечером в течение 12 месяцев.
Composition
Composition (per 1 tablet)
Active ingredients:
affinity-purified antibodies to endothelial nitric oxide synthase - 10,000 EMU*,
affinity-purified antibodies to prostate-specific antigen - 10,000 EMU*.
Excipients: lactose monohydrate, microcrystalline cellulose, magnesium stearate.
• EMU - units of modifying action.
Tablets of a flat-cylindrical shape, with a score and bevel, from white to almost white in color.
On the flat side with the score, the inscription MATERIA MEDICA is applied, and on the other flat side, the inscription AFALAZA is applied.

Таблетки плоскоцилиндрической формы, с риской и фаской, от белого до почти белого цвета.
На плоской стороне с риской нанесена надпись MATERIA MEDICA, на другой плоской стороне нанесена надпись AFALAZA.
Contraindications & warnings

Age under 18 years.
Lactase deficiency, hereditary galactose intolerance, glucose-galactose malabsorption.
Use during pregnancy and breastfeeding
The drug Afalaza® is not intended for use in women.
Patients with rare hereditary galactose intolerance, lactase deficiency, or glucose-galactose malabsorption should not take this medication.
Effect on the ability to drive vehicles and operate machinery
Afalaza® does not affect the ability to drive vehicles or operate other potentially dangerous machinery.

Pharmacology
It has been experimentally and clinically demonstrated that antibodies to prostate-specific antigen (PSA) affinity-purified modify the functional activity of endogenous PSA, altered in benign prostatic hyperplasia, which is accompanied by an enhancement of the regulatory influence of this antigen on functional and metabolic processes in prostate tissue, exhibiting pronounced anti-inflammatory and anti-edema effects.
Antibodies to endothelial nitric oxide synthase, affinity-purified, contribute to an increase in blood flow velocity, including in the vessels of the penis and prostate; they provide a protective effect on the endothelium (helping to reduce vascular reactivity, decrease vascular spasm, and improve peripheral microcirculation).
Clinically, the anti-inflammatory effect of antibodies to PSA has been shown in the treatment of chronic abacterial prostatitis. For antibodies to endothelial nitric oxide synthase, clinical efficacy has been demonstrated in the treatment of erectile dysfunction.
The combined use of components in the complex preparation Afalaza® is accompanied by a synergistic effect: antibodies to endothelial nitric oxide synthase, due to their endothelial protective action and improved vascularization, enhance the antiproliferative and anti-inflammatory activity of antibodies to PSA. The synergistic effect is likely also due to nonspecific mechanisms that enhance intracellular signal transduction through dilutions of antibodies to endothelial nitric oxide synthase.
In patients with benign prostatic hyperplasia (BPH), the drug, when used for 12 months, contributes to a reduction in prostate volume, improvement in urodynamic parameters (decrease in residual urine volume, increase in maximum urine flow rate), and a significant reduction in the severity of dysuric disorders, without affecting PSA levels. The drug helps improve the quality of life for patients with BPH.
The sensitivity of modern physicochemical analysis methods (gas-liquid chromatography, high-performance liquid chromatography, chromatographic mass spectrometry) does not allow for the assessment of the content of active substances of the Afalaza® preparation in biological fluids, organs, and tissues, which makes it technically impossible to study pharmacokinetics.

Экспериментально и клинически показано, что антитела к простатоспецифическому антигену (ПСА) аффинно очищенные модифицируют функциональную активность эндогенного ПСА, измененную при доброкачественной гиперплазии предстательной железы, что сопровождается усилением регуляторного влияния данного антигена на функциональные и метаболические процессы в ткани предстательной железы, оказывают выраженное противовоспалительное и противоотечное действие.
Антитела к эндотелиальной NO синтазе аффинно очищенные способствуют увеличению скорости кровотока, в том числе в сосудах полового члена и предстательной железы; оказывают протективное действие по отношению к эндотелию (способствуют снижению реактивности сосудов, уменьшению сосудистого спазма и улучшению периферической микроциркуляции).
Клинически показано противовоспалительное действие антител к ПСА при лечении хронического абактериального простатита. Для антител к эндотелиальной NO синтазе была показана клиническая эффективность в лечении эректильной дисфункции.
Совместное применение компонентов в составе комплексного препарата Афалаза® сопровождается синергическим эффектом: антитела к эндотелиальной NO синтазе за счет эндотелиопротективного действия и улучшения васкуляризации усиливают антипролиферативную и противовоспалительную активность антител к ПСА. Синергический эффект обусловлен, вероятно, также неспецифическими механизмами усиления внутриклеточной трансдукции сигнала разведениями антител к эндотелиальной NO синтазе.
У пациентов с доброкачественной гиперплазией предстательной железы (ДГПЖ) препарат при применении в течение 12 месяцев способствует уменьшению объема предстательной железы, улучшению показателей уродинамики (снижение объема остаточной мочи, увеличение максимальной скорости мочи), значимому снижению выраженности дизурических расстройств, не оказывая влияния на уровень ПСА. Препарат способствует улучшению качества жизни пациентов с ДГПЖ.
Чувствительность современных физико-химических методов анализа (газожидкостная хроматография, высокоэффективная жидкостная хроматография, хромато-масс-спектрометрия) не позволяет оценивать содержание действующих веществ препарата Афалаза® в биологических жидкостях, органах и тканях, что делает технически невозможным изучение фармакокинетики.
