Печеночная энцефалопатия, печеночная прекома и кома, гипераммониемия.

How to use

For oral use during meals, mixing with various beverages (water, tea, coffee, juices, etc.) or liquid food.
For constipation and dysbiosis. The preparation should be taken once, the entire daily dose at once, in a single intake.
Children 1-6 years — 2.5-5 g (1/2 - 1 teaspoon of powder) per day.
Children 6-12 years — 5-10 g (1 - 2 teaspoons of powder) per day.
Children 12-16 years — 10-20 g (2 - 4 teaspoons of powder) per day.
Adults: 20 g (4 teaspoons of powder) per day.
The preparation should induce a bowel movement once a day; after achieving a stable laxative effect over several days of use, the dose can be reduced. In some cases, half of the recommended daily dose may be sufficient to achieve the desired effect. The minimum course of therapy, after which a consultation with a doctor should be sought, is 5 months. The use of the preparation in children is recommended to be conducted under medical supervision.
For stool regulation for medical purposes. 20 g of the preparation, diluted in 1 L of water, is taken three times (at 12, 15, and 18 hours) on the eve of medical procedures.
In hepatic encephalopathy, hepatic precoma and coma, hyperammonemia. The dose of the preparation should be individually adjusted for each patient, depending on the severity of the disease and individual response to the preparation. The initial daily dose is usually 0.5-0.7 g per 1 kg of body weight. This dose is divided into three single doses. Taken during meals. A dose should be selected that induces two "soft" stools per day.

Внутрь во время еды, смешивая с различными напитками (вода, чай, кофе, соки и др.) или жидкой пищей.
При запоре и дисбактериозе. Препарат следует принимать однократно, всю суточную дозу сразу, за один прием.
Дети 1 -6 лет —2,5 - 5 г (1/2 - 1 чайная ложка порошка) в сутки.
Дети 6 -12 лет —5 - 10 г (1 - 2 чайных ложки порошка) в сутки.
Дети 12 -16 лет —10 - 20 г (2 - 4 чайных ложки порошка) в сутки.
Взрослые: 20 г (4 чайных ложки порошка) в сутки.
Препарат должен вызывать стул один раз в сутки; после наступления стабильного слабительного эффекта в течение нескольких дней применения препарата, его дозу можно снизить. В ряде случаев для получения необходи-мого эффекта достаточной бывает половины рекомендованной суточной дозы. Минимальный курс терапии, после которого следует обратиться за консультацией к врачу —5 месяцев. Применение препарата у детей рекомендуется проводить под наблюдением врача.
При регулировании стула в медицинских целях. По 20 г препарата, разведенного в 1 л воды, принимают три раза (в 12, 15 и 18 часов) накануне проведения медицинских процедур.
При печеночной энцефалопатии, печеночной прекоме и коме, гипераммониемии. Доза препарата должна быть подобрана индивидуально для каждого пациента, в зависимости от степени выраженности заболевания и индивиду-альной реакции на препарат. Contraindications & warnings

Individual intolerance to lactitol; galactosemia; intestinal obstruction; suspicion of organic damage to the gastrointestinal tract; abdominal pain and rectal bleeding of unclear origin.

Lactitol has a low caloric content (2 kcal/g), does not affect blood glucose levels, and can be used in patients with diabetes mellitus.
Throughout the course of treatment, increased fluid intake is recommended.
The use of Exportal® does not affect psychomotor functions related to driving vehicles or operating machinery and equipment.

Индивидуальная непереносимость лактитола, галактоземия, непроходимость кишечника, подозрение на органиче-ские повреждения желудочно-кишечного тракта, абдоминальные боли и ректальные кровотечения неясного генеза.

Лактитол имеет низкую калорийность (2 ккал/г), не влияет на уровень глюкозы в крови и может применяться у больных сахарным диабетом.
На протяжении всего курса применения препарата желательно повышенное потребление жидкости.
Применение Экспортала® не оказывает влияния на психомоторные функции, связанные с вождением автомобиля или управлением машинами и механизмами.

Side effects, overdose & interactions

At the beginning of treatment, there may be a sensation of discomfort in the abdomen and flatulence. These phenomena usually disappear with continued use of the medication as the body adapts to it.
In the case of prolonged use of increased doses for the treatment of hepatic encephalopathy in the patient, due to diarrhea, an electrolyte imbalance may develop.

The signs of overdose include diarrhea; in rare cases, vomiting may occur. These symptoms are usually resolved by reducing the dosage of the medication.

When taking Exportal® simultaneously with enteric-coated pH-dependent release medications, it should be noted that lactitol lowers intestinal pH.
It is not recommended to use Exportal® within two hours after taking other medications.

В начале лечения возможно ощущение дискомфорта в животе, метеоризм. Эти явления, как правило, исчезают при продолжении применения препарата, по мере адаптации к нему.
В случае применения повышенных доз в течение длительного времени при лечении печеночной энцефалопатии у пациента, вследствие диареи, может развиться электролитный дисбаланс.

Признаком передозировки является понос, в редких случаях возможна рвота. Эти явления обычно устраняются снижением дозы препарата.

При одновременном приеме Экспортала® с кишечнорастворимыми препаратами рН-зависимого высвобождения следует учитывать, что лактитол понижает рН кишечника.
Не рекомендуется применять Экспортал® в течение двух часов после приема других лекарственных средств.

Pharmacology

In the large intestine, lactitol is broken down by intestinal flora into low-molecular-weight organic acids, leading to an increase in osmotic pressure in the large intestine, an increase in the volume of fecal matter, its softening, facilitation of defecation, and normalization of bowel function. The laxative effect usually occurs within 24 hours after administration of the drug (the delay is due to the passage of the drug through the gastrointestinal tract). At the beginning of the treatment course, a delay in the laxative action of the drug may occur, and the effect may be observed on the second or third day of its use.
In patients with hepatic encephalopathy or hepatic coma (or precoma), the effect is achieved through the migration of ammonia from the blood to the large intestine (due to the decrease in pH, increase in osmotic pressure in the intestinal lumen), removal of retained ammonium ions and other nitrogen-containing toxic substances from the large intestine. Lactitol suppresses proteolytic bacteria and increases the number of acidophilic bacteria.

Lactitol exerts its effect only in the large intestine, where it is completely delivered after oral administration. It is absorbed in minimal amounts in unchanged form (no more than 0.5 - 2% of the administered dose).

В толстом кишечнике лактитол расщепляется под действием флоры кишечника на низкомолекулярные органические кислоты, что приводит к повышению осмотического давления в толстом кишечнике, увеличению объема каловых масс, их размягчению, облегчению дефекации и нормализации работы кишечника. Слабительный эффект обычно наступает в течение 24 часов после применения препарата (отсроченность обусловлена прохождением препарата через желудочно-кишечный тракт). В начале курса лечения возможна задержка слабительного действия препарата, эффект может наступить на второй или третий день его применения.
У больных с печеночной энцефалопатией или печеночной комой (или прекомой) эффект достигается за счет миграции аммиака из крови в толстый кишечник (благодаря снижению РН, увеличению осмотического давления в просвете кишечника), удаления из толстой кишки задержанных ионов аммония и других азотосодержащих токсических веществ. Лактитол подавляет протеолитические бактерии и увеличивает количество ацидофильных бактерий.

Лактитол осуществляет свой эффект только в толстом кишечнике, куда он полностью попадает после приема внутрь. Он абсорбируется в минимальных количествах в неизмененном виде (не более 0,5 - 2 % от принятой дозы).
