Indications

Treatment of pathologically increased tone and spasms of striated muscle, caused by organic neurological pathology (including damage to the pyramidal tracts, multiple sclerosis, stroke, myelopathy, encephalomyelitis).
Treatment of increased tone and muscle spasms, muscle contractures accompanying musculoskeletal disorders (spondylosis, spondyloarthritis, osteoarthritis of large joints, cervical and lumbar pain syndromes).
Rehabilitation treatment after surgical interventions in orthopedics and traumatology.
As part of combined therapy for obliterating vascular diseases (obliterating atherosclerosis, diabetic angiopathy, obliterating thromboangiitis, Raynaud's disease), as well as diseases arising from vascular innervation disorders (acrocyanosis, intermittent angioneurotic dysbasia).

How to use

Orally. For adults, the initial dose is 50 mg 2-3 times a day, gradually increasing to 150 mg 2-3 times a day.
The duration of the treatment course depends on the severity and nature of the disease.

1 tablet contains:
active ingredient: tolperisone hydrochloride - 50 mg or 150 mg;
excipients: citric acid monohydrate, lactose monohydrate, hypromellose (hydroxypropyl cellulose), croscarmellose sodium, stearic acid, calcium stearate;
excipients for the coating: hypromellose (hydroxypropyl methylcellulose), titanium dioxide, macrogol 6000 (polyethylene glycol 6000).

Tablets coated with a film, white or almost white in color, biconvex, elongated shape with rounded ends, with a score line.

вспомогательные вещества: лимонной кислоты моногидрат, лактозы моногидрат, гипролоза (гидроксипропилцеллюлоза), кросповидон, кислота стеариновая, кальция стеарат;

Таблетки, покрытые пленочной оболочкой белого или почти белого цвета, двояковыпуклые, продолговатой формы со скругленными концами, с риской.

Contraindications & warnings

Myasthenia gravis; age under 18 years; increased sensitivity to the components of the drug; pregnancy; lactation period. Due to the presence of lactose in the composition, patients with lactose intolerance, lactase deficiency, or glucose-galactose malabsorption should not take the drug.

During treatment, caution should be exercised when driving vehicles and engaging in other potentially dangerous activities that require increased attention and quick psychomotor reactions.

Миастения, возраст до 18 лет, повышенная чувствительность к компонентам препарата, беременность, период лактации. В связи с наличием в составе лактозы, пациенты с непереносимостью лактозы, дефицитом лактазы или глюкозо-галактозной мальабсорбцией не должны принимать препарат.

В период лечения необходимо соблюдать осторожность при вождении автотранспорта и занятии другими потенциально опасными видами деятельности, требующими повышенной концентрации внимания и быстроты психомоторных реакций.

Side effects, overdose & interactions

Muscle weakness, headache, arterial hypotension, nausea, vomiting, gastralgia (usually resolves with dose reduction); rarely – hypersensitivity reactions (skin itching, erythema, urticaria, angioedema, bronchospasm, anaphylactic shock).

Symptoms: respiratory and cardiac depression, decreased blood pressure.
Treatment: symptomatic.

Tolperisone acts on the central nervous system but does not cause a sedative effect, therefore it can be used in combination with sedatives, hypnotics, and tranquilizers. It does not enhance the effect of alcohol on the central nervous system. It increases the potency of niflumic acid, so when used together, the dose of the latter can be reduced. General anesthetics, peripheral muscle relaxants, psychoactive medications, and clonidine enhance the effect.

Мышечная слабость, головная боль, артериальная гипотензия, тошнота, рвота, гастралгия (обычно проходят при снижении дозы), редко – реакции гиперчувствительности (кожный зуд, эритема, крапивница, ангионевротический отек, бронхоспазм, анафилактический шок).

Симптомы: угнетение дыхания и сердечной деятельности, снижение артериального давления.
Лечение: симптоматическое.

Толперизон оказывает действие на центральную нервную систему, но не вызывает седативного эффекта, поэтому может применяться в комбинации с седативными, снотворными средствами, транквилизаторами. Не усиливает действие алкоголя на центральную нервную систему. Увеличивает выраженность действия нифлумовой кислоты, поэтому при их одновременном применении можно снизить дозу последней. Лекарственные средства для общей анестезии, периферические миорелаксанты, психоактивные лекарственные средства, клонидин усиливают эффект.

Pharmacology

Central acting muscle relaxant. It exerts a membrane-stabilizing effect, inhibits the conduction of nerve impulses in primary afferent fibers and motor neurons, leading to the blockade of spinal mono- and polysynaptic reflexes. It likely mediates the blockade of mediator release by inhibiting the influx of Ca+2 into synapses. It inhibits the conduction of excitation along the reticulospinal pathway in the brainstem.

Regardless of the influence on the central nervous system, it enhances peripheral blood flow. A weak spasmolytic and antiadrenergic effect of tolperisone may play a role in the development of this effect.

After oral administration, tolperisone is well absorbed from the small intestine. The maximum concentration in plasma is reached 30 minutes to 1 hour after administration, with a bioavailability of approximately 20% due to significant presystemic metabolism. Tolperisone is extensively metabolized in the liver and kidneys. It is excreted almost exclusively (over 99%) in the urine as metabolites. The pharmacological activity of the metabolites is unknown.

Миорелаксант центрального механизма действия. Оказывает мембраностабилизирующее действие, тормозит проведение нервных импульсов в первичных афферентных волокнах и двигательных нейронах, что приводит к блокированию спинномозговых моно- и полисинаптических рефлексов. Вероятно, опосредует блокирование выделения медиаторов путем торможения поступления Ca+2 в синапсы. Тормозит проведение возбуждения по ретикулоспинальному пути в стволе мозга.
Независимо от влияния центральной нервной системы усиливает периферический кровоток. В развитии этого эффекта может играть роль слабый спазмолитический и антиадренергический эффект толперизона.

После приема внутрь толперизон хорошо всасывается из тонкой кишки. Максимальная концентрация в плазме крови достигается через 30 мин- 1 ч после приема, биодоступность из-за выраженного пресистемного метаболизма составляет около 20 %. Толперизон экстенсивно метаболизируется в печени и почках. Экскретируется с мочой почти исключительно (более 99 %) в виде метаболитов. Фармакологическая активность метаболитов неизвестна
