Indications

Diseases caused by disturbances in calcium and phosphorus metabolism due to insufficient endogenous synthesis of 1,25-dihydroxyvitamin D3:
- osteoporosis (including postmenopausal, senile, steroid-induced, etc.);
- osteodystrophy in chronic kidney disease (CKD);
- hypoparathyroidism and pseudohypoparathyroidism;
- vitamin D-resistant rickets and osteomalacia.

excipients: peanut oil 98.8000 mg, anhydrous citric acid 0.01500 mg, propyl gallate 0.02000 mg, ?-tocopherol 0.02000 mg, ethanol 1.14400 mg;
soft gelatin capsule: anhydrosorb 12.39 mg, glycerin 85% 18.67 mg, gelatin 77.80 mg, yellow iron oxide dye 0.08 mg, titanium dioxide 1.06 mg.

Soft gelatin capsules of oval shape, light yellow in color. The contents of the capsules are a clear colorless or light yellow to yellow-brown oily liquid.

Мягкие желатиновые капсулы овальной формы светло-желтого цвета. Содержимое капсул - прозрачная бесцветная или от светло-желтого до желто-коричневого цвета маслянистая жидкость.

Pharmacology

Alfacalcidol (1α-hydroxyvitamin D3) is rapidly converted in the liver to 1,25-dihydroxyvitamin D3, the active metabolite of vitamin D (calcitriol), which acts as a regulator of calcium and phosphorus metabolism. It increases the absorption of calcium and phosphorus in the intestine, enhances their reabsorption in the kidneys, restores positive calcium balance in the treatment of calcium malabsorption syndrome, and reduces serum parathyroid hormone levels.

By affecting both parts of the bone remodeling process—(resorption and synthesis)—alfacalcidol not only increases the mineralization of bone tissue but also enhances its elasticity by stimulating the synthesis of bone matrix proteins, bone morphogenetic proteins, and bone growth factors, which contributes to a reduction in the frequency of fractures.

In elderly patients with endocrine-immune dysfunction, including a deficiency in D-hormone (calcitriol) production, there is a decrease in overall muscle mass (sarcopenia) and the onset of muscle weakness syndrome (due to impaired normal functioning of the neuromuscular apparatus), which is accompanied by an increased risk of falls and related injuries and fractures. Several studies have shown a significant reduction in the frequency of falls among elderly patients when using alfacalcidol. Alfacalcidol stimulates the regeneration of muscle fibers, restoring lost muscle tone.

Alfacalcidol is fat-soluble, and its bioavailability when taken orally is approximately 100%. After absorption, alfacalcidol is metabolized in the liver to form the active metabolite 1,25-dihydroxyvitamin D3. The concentration of 1,25-dihydroxyvitamin D3 in plasma reaches its peak 8-12 hours after a single dose of alfacalcidol; the half-life of 1,25-dihydroxyvitamin D3 is about 35 hours. It is excreted by the kidneys and in bile in approximately equal amounts.

Альфакальцидол (1?-гидроксивитамин D3) быстро превращается в печени в 1,25-дигидроксивитамин D3, - активный метаболит витамина D (кальцитриол), который действует как регулятор обмена кальция и фосфора. Повышает абсорбцию кальция и фосфора в кишечнике, увеличивает их реабсорбцию в почках, восстанавливает положительный кальциевый баланс при лечении синдрома кальциевой мальабсорбции и снижает концентрацию в крови паратиреоидного гормона.
Воздействуя на обе части процесса костного ремоделирования-(резорбцию и синтез), альфакальцидол не только увеличивает минерализацию костной ткани, но и повышает ее упругость за счет стимулирования синтеза белков матрикса кости, костных морфогенетических белков, факторов роста кости, что способствует уменьшению частоты развития переломов.
У пациентов пожилого возраста на фоне эндокринно-иммунной дисфункции, в том числе дефицита продукции D-гормона (кальцитриола), происходит снижение общей мышечной массы (саркопения) и появление синдрома мышечной слабости (вследствие нарушения нормального функционирования нервно-мышечного аппарата), что сопровождается повышением риска падений и обусловленных этим травм и переломов. В ряде исследований было показано значительное снижение частоты падений пожилых пациентов при применении альфакальцидола. Альфакальцидол стимулирует регенерацию мышечных волокон, что восстанавливает утраченный мышечный тонус.

Альфакальцидол жирорастворим и его биодоступность при приеме внутрь - около 100 %. После всасывания альфакальцидол метаболизируется в печени с образованием активного метаболита 1,25-дигидроксивитамина D3. Концентрация 1,25-дигидроксивитамина D3 в плазме крови достигает максимума через 8-12 часов после однократной дозы альфакальцидола; период полувыведения 1,25-дигидроксивитамина D3 около 35 часов. Выводится почками и с желчью примерно в одинаковом количестве.
