INSERTION
The insertion of the pessary is performed in accordance with generally accepted aseptic rules.
• After examining the woman with an empty bladder, position the obstetric pessary so that the wide base is at the entrance to the vagina. Insert the lower semicircle of the wide base into the vagina.
• Then, pressing on the back wall of the vagina, insert the upper semicircle of the wide base. After that, fully insert the obstetric pessary.
• Rotate the obstetric pessary so that the wide base is located in the posterior fornix of the vagina, and the cervix is in the central opening of the obstetric pessary.
REMOVAL OF THE OBSTETRIC PESSARY
The obstetric pessary is planned to be removed at 37-38 weeks of pregnancy in a hospital setting.
The technique for removal is the reverse of insertion.
After the removal of the obstetric pessary, it is advisable to sanitize the genital tract depending on the nature of the vaginal microflora.

APPLICATION AREA – obstetrics. Obstetric pessaries are used in specialized medical institutions by a qualified physician.
The shape of the device corresponds to the anatomical structure of the female reproductive organs and ensures reliable fixation of the device inside the vagina. The pessary resembles a trapezoid with concave sides and rounded corners. The narrow side of the trapezoid rests against the pubic symphysis, while the wide part of the trapezoid "envelops" the rectum, not causing problems during defecation. There are several functional openings: a large central opening for the cervix and lateral openings for unobstructed drainage of vaginal secretions.
The selection of the size of the obstetric pessary is performed by a physician individually for each woman - they must correspond to the anatomical features of the reproductive organs.

The obstetric unloading pessary "Junona" is produced in three sizes:
1 - type is used for the earliest manifestations of uterine prolapse or mildly expressed pathologies of the reproductive organs. Typically, such conditions are manifestations of mild functional disorders. The outer diameter of the device is 60.5-65.5 mm. This type of pessary is the smallest in the presented range of products.
2 - type has larger dimensions. It is used for the correction of moderately expressed disorders associated with genital prolapse. It is prescribed for pathologies accompanied by 2-3 degrees of severity. The outer diameter of the device is 65.5-70.5 mm.
3 - type can be used for pronounced pathologies of the intimate area. The outer diameter of the device is 70.5-75.5 mm.
PRECAUTIONS
The use of the obstetric pessary may lead to increased vaginal secretion.
The obstetric pessary is not intended for the correction of genital prolapse.
The use of the obstetric pessary is not permissible if the integrity of the final packaging is compromised or if the expiration date indicated on the packaging has been exceeded.
When using an obstetric pessary that does not correspond to the anatomical features of the patient, the development of pain syndrome may occur. In this case, removal of the pessary and replacement with a properly sized obstetric pessary is required.
MECHANISM OF ACTION
The mechanism of action of the obstetric unloading pessary is based on reducing the load on the cervix due to decreased pressure from the fetal sac.
Isthmic-cervical insufficiency: the external and internal os are dilated, and the fetal membranes prolapse into the cervical canal.
The obstetric pessary is installed: the cervix is positioned in the central opening of the pessary, the large base is located in the posterior fornix of the vagina, and the small base is in the anterior fornix. The lateral surface of the pessary contacts the vaginal walls, which envelop and hold it. Pressure on the cervix is reduced.
RECOMMENDATIONS FOR PATIENTS
When using the obstetric unloading pessary, regular monitoring by an obstetrician-gynecologist is necessary; it is advisable to maintain sexual rest, and increased discharge from the genital tract may occur (inflammation must be excluded).
