How to use

The medication is prescribed at a dosage of 600 mg (2 tablets) once a day.
Tablets should be taken in the morning on an empty stomach, approximately 30 minutes before the first meal, without chewing and with a small amount of liquid. The duration of the treatment course is determined by the doctor.

Contraindications & warnings

- Pregnancy;
- Breastfeeding period (insufficient experience with the use of the drug);
- Pediatric age (efficacy and safety have not been established);
- Increased sensitivity to the components of the drug.

In patients with diabetes mellitus, constant monitoring of blood glucose levels is required during treatment with the drug (especially in the initial stages of therapy). In some cases, it may be necessary to reduce the dose of insulin or oral hypoglycemic agents to avoid the development of hypoglycemia.
The patient should be informed that while taking Tiolepta, it is advisable to refrain from consuming ethanol.

— повышенная чувствительность к компонентам препарата.

У пациентов с сахарным диабетом во время лечения препаратом необходим постоянный контроль концентрации глюкозы в крови (особенно на начальной стадии терапии). В некоторых случаях следует уменьшить дозу инсулина или перорального гипогликемического препарата, чтобы избежать развития гипогликемии.
Пациент должен быть информирован о том, что при приеме Тиолепты следует воздержаться от употребления этанола.

Side effects, overdose & interactions

Gastrointestinal side effects: possible - dyspepsia, nausea, vomiting, heartburn.
Metabolic side effects: possible - development of hypoglycemia (due to improved glucose absorption).
Allergic reactions: in rare cases - skin manifestations (including urticaria).
The medication is usually well tolerated by patients.

When used simultaneously, the drug may reduce the effect of cisplatin (thioctic acid reacts with metal ion complexes). For the same reason, when taking Tiolepta in the morning, iron and magnesium preparations, as well as dairy products (due to their calcium content), are recommended to be taken in the afternoon.
When used simultaneously, the drug enhances the action of insulin and oral hypoglycemic agents.
Ethanol reduces the therapeutic activity of thioctic acid.

Со стороны пищеварительной системы: возможно - диспепсия, тошнота, рвота, изжога.
Со стороны обмена веществ: возможно - развитие гипогликемии (в связи с улучшением усвоения глюкозы).
Аллергические реакции: в отдельных случаях - кожные проявления (в т.ч. крапивница).
Обычно препарат хорошо переносится пациентами.

При одновременном применении препарат может снижать эффект цисплатина (тиоктовая кислота реагирует с ионными комплексами металлов). По этой же причине при приеме Тиолепты утром препараты железа, магния, а также молочные продукты (из-за содержания в них кальция) рекомендуется принимать во второй половине дня.
При одновременном применении препарат усиливает действие инсулина и пероральных гипогликемических средств.
Этанол снижает терапевтическую активность тиоктовой кислоты.

Pharmacology

Metabolic agent, it is an endogenous antioxidant that binds free radicals.
Thioctic acid (alpha-lipoic acid) is formed in the body during the oxidative decarboxylation of alpha-keto acids. As a coenzyme of mitochondrial multienzyme complexes, it participates in the oxidative decarboxylation of pyruvic acid and alpha-keto acids.
It has hepatoprotective, hypolipidemic, hypocholesterolemic, and hypoglycemic effects. It contributes to the reduction of blood glucose levels and the increase of glycogen content in the liver, as well as overcoming insulin resistance. It is involved in the regulation of lipid and carbohydrate metabolism, stimulates cholesterol metabolism, and improves liver function. In terms of biochemical action, thioctic acid is similar to B vitamins.

Absorption
After oral administration, it is rapidly and completely absorbed from the gastrointestinal tract. Concurrent food intake reduces the absorption of the drug. Bioavailability is 30%. Tmax - 40-60 minutes.
Distribution
It has a "first-pass effect" through the liver. Vd - approximately 450 ml/kg.
Metabolism
Metabolite formation occurs as a result of side chain oxidation and conjugation.
Excretion
80-90% of thioctic acid and its metabolites are excreted in the urine. T1/2 - 20-50 minutes. Total plasma clearance is 10-15 ml/min.

Метаболический препарат, представляет собой эндогенный антиоксидант, связывающий свободные радикалы.
Тиоктовая кислота (альфа-липоевая кислота) образуется в организме при окислительном декарбоксилировании альфа-кетоксилот. В качестве кофермента митохондриальных мультиферментных комплексов участвует в окислительном декарбоксилировании пировиноградной кислоты и альфа-кетокислот.
Оказывает гепатопротекторное, гиполипидемическое, гипохолестеринемическое, гипогликемическое действие. Способствует снижению содержания глюкозы в крови и увеличению содержания гликогена в печени, а также преодолению инсулинорезистентности. Участвует в регулировании липидного и углеводного обмена, стимулирует обмен холестерина, улучшает функцию печени. По характеру биохимического действия тиоктовая кислота близка к витаминам группы В.

Всасывание
После приеме внутрь быстро и полностью всасывается из ЖКТ. Одновременный прием пищи снижает всасывание препарата. Биодоступность составляет 30%. Тmax - 40-60 мин.
Распределение
Обладает "эффектом первого прохождения через печень". Vd - около 450 мл/кг.
Метаболизм
Образование метаболитов происходит в результате окисления боковой цепи и конъюгирования.
Выведение
Выводятся с мочой 80-90% тиоктовой кислоты и ее метаболитов . T1/2 - 20-50 мин. Общий плазменный клиренс составляет 10-15 мл/мин.
