How to use

The obstetric dome-shaped pessary is installed by a physician during an outpatient visit or in a hospital. Self-installation of the pessary by the patient is not possible. Before installation, the device must be disinfected in a water solution of chlorhexidine. It is also recommended to conduct a microscopy analysis of vaginal secretions in advance to identify hidden infections.

In cases of cervical shortening or dilation of the internal os, the obstetric pessary is installed at 15-20 weeks of pregnancy. The pessary does not close (occlude) the cervix but rather supports it and shifts it towards the sacrum, preventing further dilation. For preventive purposes, the obstetric pessary may be installed in women at risk of pregnancy loss (history of preterm labor, multiple pregnancies) between 13-16 weeks.

Акушерский куполообразный пессарий устанавливается врачом на амбулаторном приёме или в стационаре. Самостоятельная установка пессария пациенткой невозможна. Перед установкой изделие должно быть продезинфицировано в водном растворе хлоргексидина. Также рекомендуется предварительно сдать анализ на микроскопию вагинального секрета, чтобы выявить скрытые инфекции.
При укорочении шейки матки или расширении внутреннего зева акушерский пессарий устанавливается в 15-20 недель беременности. Пессарий не закрывает (смыкает) шейку матки, а скорее, поддерживает её и перемещает в сторону крестца, что предотвращает её дальнейшее раскрытие. В профилактических целях акушерский пессарий может быть установлен у женщин группы риска по невынашиванию беременности (преждевременные роды в анамнезе, многоплодная беременность) в сроках 13-16 недель.

Pharmacology

Obstetric pessary is a device used in pregnant women with cervical insufficiency (CI) when diagnosing the threat of preterm labor. It is used to maintain the uterus in the necessary natural position, as well as to support a shortened cervix that is at risk of further dilation. This conservative method of correcting CI has been used in Russia for over 18 years (in some countries for over 30) and has a high effectiveness rate of over 85%. The selection and installation of the obstetric pessary is performed only by a qualified obstetrician-gynecologist. Installation can be done both in a hospital setting and on an outpatient basis.

The dome-shaped obstetric pessary is the most commonly used type of obstetric pessary, which has the shape of a deep cup, a large central opening for fixation on the cervix, and small functional openings for the drainage of vaginal secretions. Like all silicone pessaries, it is elastic, can be bent, and can be inserted completely painlessly.

Obstetric pessaries are available in 13 sizes, which differ in their outer diameter (65 mm or 70 mm), as well as in the height of curvature (each 17 mm, 21 mm, 25 mm, 30 mm). The inner diameter for all models is either 32 mm or 35 mm. Higher models are preferred for more serious conditions.

The dome-shaped pessary is recommended for continuous use. In the case of pregnancy, it is installed once and performs its functions until the term of full-term pregnancy, that is, until 36-37 weeks. The pessary is removed by a doctor during an appointment.

Применяется для удержания матки в необходимом естественном положении, а также для фиксации укороченной шейки матки с угрожающим дальнейшим раскрытием. Данный консервативный метод коррекции ИЦН применяется в России уже более 18 лет (в ряде стран - более 30) и обладает высокой эффективностью - более 85%. Подбор акушерского пессария и его установку проводит только врач акушер-гинеколог. Установка возможна как в условиях стационара, так и амбулаторно.
Куполообразный акушерский пессарий - наиболее используемый вид акушерского пессария, который имеет форму глубокой чаши, большое центральное отверстие для фиксации на шейке матки и маленькие функциональные отверстия для оттока влагалищного секрета. Подобно всем силиконовым пессариям он является упругим, его можно согнуть и таким образом вставить совершенно без боли.
