How to use

Locally. For intravaginal use only. Vaginal tablets are to be inserted in the evening into the vagina, as deeply as possible while lying on the back with slightly bent legs, daily for 6 days at a dose of 1 vaginal tablet of 100 mg. A repeat course of treatment is possible after consulting a doctor.
For sanitization of the birth canal before delivery, a single insertion of one tablet is recommended.
Instructions for using the applicator:
1. Extend the plunger of the applicator to the stop. Place one vaginal tablet in the applicator;
2. Carefully insert the applicator with the tablet as deeply as possible into the vagina (best in a lying position on the back with slightly bent legs);
3. Insert the tablet into the vagina by slowly pressing the plunger of the applicator to the stop;
4. Remove the applicator;
5. After use and before reusing, fully withdraw the plunger from the applicator, wash the plunger and applicator with warm (not hot) soapy water, and dry thoroughly.

Местно. Только для интравагинального применения. Вагинальные таблетки вводят вечером во влагалище, как можно глубже лежа на спине при слегка согнутых ногах, ежедневно в течение 6 дней по 1 вагинальной таблетке 100 мг. Повторный курс лечения возможен после консультации врача.
Для санации родовых путей перед родами рекомендуется однократное введение одной таблетки.
Инструкции по использованию аппликатора:
1. Выдвинуть поршень аппликатора до упора. Поместить одну вагинальную таблетку в
аппликатор;
2. Осторожно вставить аппликатор с таблеткой максимально глубоко во влагалище (лучше
всего в положении лежа на спине при слегка согнутых ногах);
3. Ввести таблетку во влагалище, медленно нажимая на поршень аппликатора до упора;
4. Извлечь аппликатор;
5. После использования и перед повторным использованием полностью извлечь поршень из
аппликатора, промыть поршень и аппликатор теплой (не горячей) мыльной водой и вытереть насухо.

Contraindications & warnings

• Hypersensitivity to clotrimazole and other components of the medication;

• First trimester of pregnancy.
Use of the medication by pregnant women and women during breastfeeding:
Contraindicated for use in the first trimester of pregnancy. The appropriateness of prescribing the medication in the second or third trimester should be determined individually after consultation with a physician. When prescribing the medication during the second or third trimesters of pregnancy, the balance between the expected benefits for the mother and the potential risks to the fetus should be considered. The use of a vaginal applicator is not recommended. Caution is advised during breastfeeding (it is unknown whether clotrimazole passes into breast milk).

Precautions and special instructions for use:

• To prevent urogenital infection, simultaneous treatment of sexual partners is necessary.

• In the case of trichomoniasis, for more effective treatment, other systemic medications (e.g., metronidazole orally) should be used in conjunction with Kandid-B6.

• In the case of simultaneous infection of the labia and adjacent areas (candidal vulvitis), local treatment with Kandi cream should be additionally administered. During pregnancy, treatment with vaginal tablets should be conducted without an applicator.

• Patients with liver insufficiency should periodically monitor liver function.

• If signs of hypersensitivity or irritation occur, treatment should be discontinued.

• If there is no effect within 6 days, the diagnosis should be confirmed.

• Use with caution during lactation.

• Avoid using tablets during menstruation.

• Should not be used in children under 12 years of age.

Применение лекарственного препарата беременными женщинами, женщинами в период грудного вскармливания:
Противопоказан к применению в I триместр беременности. Вопрос о целесообразности назначения препарата во II или III триместре должен решаться индивидуально после консультации с врачом. При назначении препарата во время II или III триместров беременности следует учитывать соотношение предполагаемой пользы от назначения препарата для матери и возможного риска для плода. Нежелательно использование вагинального аппликатора. С осторожностью - в период грудного вскармливания (неизвестно, проникает ли клотримазол в грудное молоко).

Меры предосторожности при применении и особые указания:

• Для предотвращения урогенитальной инфекции необходимо одновременное лечение половых партнеров.

• При трихомониазе для более успешного лечения совместно с Кандидом-В6 следует применять другие лекарственные средства, обладающие системным действием (например, метронидазол внутрь).

• При одновременной инфекции половых губ и прилегающих участков (кандидозный вульвит) следует дополнительно проводить местное лечение препаратом Кандид крем.
В период беременности лечение вагинальными таблетками следует проводить без аппликатора.

• У больных с печеночной недостаточностью следует периодически контролировать функциональное состояние печени.

• При проявлении признаков гиперчувствительности или раздражения лечение прекращают.

• При отсутствии эффекта в течение 6 дней следует подтвердить диагноз.

• С осторожностью - период лактации.

• Следует избегать применения таблеток в период менструации.

• Не следует применять у детей до 12 лет.

Side effects, overdose & interactions

Itching, burning, and swelling of the vaginal mucosa; vaginal discharge; headache; gastralgia; frequent urination; intercurrent cystitis; burning sensation in the partner's penis; pain during sexual intercourse.

The use of the drug in increased doses does not cause any life-threatening reactions or conditions. In the case of unintended use of the drug (orally), the following symptoms may occur: anorexia, nausea, vomiting, abdominal pain, liver dysfunction; rarely - drowsiness, hallucinations, pollakiuria, skin allergic reactions.
There is no specific antidote. Activated charcoal should be taken orally. Symptomatic treatment is required if necessary.

When administered vaginally, Clotrimazole reduces the effectiveness of Amphotericin B and other polyene antibiotics. When used simultaneously with Nystatin, the activity of Clotrimazole may be reduced.

Применение препарата в повышенных дозах не вызывает каких-либо реакций или состояний, опасных для жизни. В случае непредусмотренного применения препарата (внутрь) возможны следующие симптомы: анорексия, тошнота, рвота, гастралгии, нарушение функции печени; редко - сонливость, галлюцинации, поллакиурия, кожные аллергические реакции.
Специфического антидота нет. Необходимо принять внутрь активированный уголь. При необходимости лечение симптоматическое.

При вагинальном введении Клотримазол снижает эффективность амфотерицина В и других полиеновых антибиотиков. При одновременном применении с нистатином активность клотримазола может снижаться.

Pharmacology

Pharmacological action - a broad-spectrum antifungal agent for local use, antibacterial, antiprotozoal, trichomonacidal. The antifungal effect of the active ingredient clotrimazole (an imidazole derivative) is associated with the disruption of ergosterol synthesis, which is a component of fungal cell membranes, altering membrane permeability and leading to subsequent cell lysis. At fungicidal concentrations, it interacts with mitochondrial and peroxidase enzymes, resulting in an increase in hydrogen peroxide concentration to toxic levels, which also contributes to the destruction of fungal cells. Clotrimazole is active against the following pathogens:

• Blastomyces dermatitidis

• Candida spp (has the same activity as nystatin)

• Coccidioides immitis

• Cryptococcus neoformans (Torula histolytica)

• Dermatophytes (Trichophyton, Microsporum, Epidermophyton)

• Histoplasma capsulatum

• Malassezia furfur

• Naegleria fowleri

• Nocardia spp

• Paracoccidioides brasiliensis

• Sporotrichum schenckii

• Effective against gram-positive bacteria - the causative agent of erythrasma Corynebacterium minutissimum, as well as Staphylococcus spp., Streptococcus spp., gram-negative bacteria - Bacteroides, Gardnerella vaginalis. At high concentrations, it is active against Trichomonas vaginalis.

When clotrimazole is administered intravaginally, absorption is 3-10% of the administered dose. High concentrations in vaginal secretions and low concentrations in the blood are maintained for 48-72 hours. It is rapidly metabolized in the liver to inactive metabolites.
