How to use

Locally. For intravaginal use only.
A full applicator with gel (approximately 5 g of the product) is inserted into the vagina in the evening, while lying on the back with slightly bent legs, daily for 6 days. A repeat course of treatment is possible after consulting a doctor.

Composition

1 g of gel contains:
active ingredient: clotrimazole 20 mg
excipients: cetyl alcohol, propylene glycol, glycerol, benzyl alcohol, ceteareth-20 emulsifying wax, carbopol 940, sodium hydroxide, chlorocresol, purified water.

Contraindications & warnings

Increased sensitivity to clotrimazole or excipients; menstrual period; first trimester of pregnancy.
With caution:
Lactation period.
Pregnancy and lactation:
Clinical and experimental studies have not established that the use of the drug during pregnancy or breastfeeding has a negative impact on the health of the woman or the fetus (child). However, the question of the appropriateness of prescribing the drug should be decided individually after consulting a doctor.

To prevent urogenital reinfection, simultaneous treatment of sexual partners is necessary.
In the case of trichomoniasis, for more effective treatment, other systemic medications (e.g., metronidazole orally) should be used in conjunction with Candido.
During pregnancy, treatment with the vaginal gel should be conducted without an applicator.
In patients with liver insufficiency, liver function should be monitored periodically.
If signs of hypersensitivity or irritation occur, treatment should be discontinued.
Avoid contact of the medication with the mucous membrane of the eyes.
Do not use the medication during menstruation.
If there is no effect within 4 weeks, the diagnosis should be confirmed.

При клинических и экспериментальных исследованиях не было установлено, что применение препарата в период беременности или в период кормления грудью оказывает отрицательное влияние на здоровье женщины или плода (ребенка). Однако вопрос о целесообразности назначения препарата должен решаться индивидуально после консультации врача.

Для предотвращения урогенитальной реинфекции необходимо одновременное лечение половых партнеров.
При трихомониазе для более успешного лечения совместно с Кандидом следует применять другие лекарственные средства, обладающие системным действием (например, метронидазол внутрь).
В период беременности лечение вагинальным гелем следует проводить без аппликатора.
У больных с печеночной недостаточностью следует периодически контролировать функциональное состояние печени.
При появлении признаков гиперчувствительности или раздражения лечение прекращают.
Избегать попадания препарата на слизистую оболочку глаз.
Не применять препарат в период менструации.
При отсутствии эффекта в течение 4 недель следует подтвердить диагноз.

Side effects, overdose & interactions

Itching, burning, hyperemia and swelling of the vaginal mucosa; vaginal discharge, headache, gastralgia, frequent urination, intercurrent cystitis, burning sensation in the penis of the sexual partner, pain in the vagina during sexual intercourse.

The use of the drug in increased doses does not cause any reactions or conditions that are life-threatening.

When administered vaginally, clotrimazole reduces the activity of amphotericin B and other polyene antibiotics. When used simultaneously with natamycin and nystatin, the activity of clotrimazole may be reduced.

Pharmacology

Candida is a broad-spectrum antifungal agent for topical use. The antifungal effect of the active ingredient clotrimazole (an imidazole derivative) is associated with the disruption of ergosterol synthesis, which is a component of fungal cell membranes, altering membrane permeability and leading to subsequent cell lysis. At low concentrations, it acts fungistatically, while at higher concentrations, it acts fungicidally, affecting not only proliferating cells. At fungicidal concentrations, it interacts with mitochondrial and peroxidase enzymes, resulting in an increase in hydrogen peroxide concentration to toxic levels, which also contributes to the destruction of fungal cells.

It is effective against dermatophytes, yeast-like and mold fungi, as well as the causative agent of tinea versicolor (Pityriasis versicolor) and the causative agent of erythrasma. It exhibits antimicrobial activity against gram-positive (staphylococci, streptococci) and gram-negative bacteria (Bacteroides, Gardnerella vaginalis), as well as against Trichomonas vaginalis.

When administered intravaginally, the absorption of clotrimazole is 3-10% of the administered dose. High concentrations in vaginal secretions and low concentrations in the blood are maintained for 48-72 hours. It is rapidly metabolized in the liver to inactive metabolites.

Антимикотический эффект активного действующего вещества клотримазола (производное имидазола) связан с нарушением синтеза эргостерина, входящего в состав клеточной мембраны грибов, что изменяет проницаемость мембраны и вызывает последующий лизис клетки. В малых концентрациях действует фунгистатически, а в больших - фунгицидно, причем не только на пролиферирующие клетки. В фунгицидных концентрациях взаимодействует с митохондриальными и пероксидазными ферментами, в результате чего происходит увеличение концентрации перекиси водорода до токсического уровня, что также способствует разрушению грибковых клеток.

Эффективен в отношении дерматофитов, дрожжеподобных и плесневых грибов, а также возбудителя разноцветного лишая (Pityriasis versicolor) и возбудителя эритразмы. Оказывает антимикробное действие в отношении грамположи- тельных (стафилококки, стрептококки) и грамотрицательных бактерий (Bacteroides, Gardnerella vaginalis), а также в отношении Trichomonas vaginalis.

При интравагинальном применении абсорбция клотримазола составляет 3-10% введенной дозы. Высокие концентрации в вагинальном секрете и низкие концентрации в крови сохраняются в течение 48-72 часов. В печени быстро метаболизируется до неактивных метаболитов.
