Indications

Used in adults and children over two years of age for the prevention and treatment of immunodeficient and toxic conditions, chronic inflammatory diseases of various etiologies. In adults, as an adjunct during vaccination against bacterial and viral infections.

How to use

Rectally. Single dose and daily dose - 100 mcg (1 suppository).
When treating cancer patients as part of a radical combined treatment regimen (chemoradiotherapy and surgery):

• once daily, for a course of 8-10 suppositories before chemoradiotherapy and surgery, with subsequent continuation of the course throughout the entire treatment period.
In patients with advanced tumor processes (stage III - IV) of various localizations as part of complex or symptomatic therapy:

• once daily, for a course of 8-10 suppositories, with a break of 15-20 days and repeating courses throughout the entire subsequent treatment period.
In children with malignant diseases of hematopoietic and lymphoid tissue:

• once daily, for a course of 10-20 suppositories. The drug should be administered throughout the entire course of chemoradiotherapy and after the completion of the course to prevent the development of toxicity.
In the complex therapy of children with laryngeal and oropharyngeal papillomatosis:

• once daily, for a course of 10 suppositories.
In opportunistic infections (cytomegalovirus and herpes infections, toxoplasmosis, chlamydia, pneumocystosis, cryptosporidiosis):

• once daily, for a course of 15-20 suppositories.
In the complex therapy of HIV infection:

• once daily, for a course of 15-20 suppositories. If necessary, repeat courses should be conducted every 2-4 weeks.
In chronic viral hepatitis and chronic brucellosis:

• once daily, for a course of 15-20 suppositories; to prevent relapse, repeat courses should be conducted every 2-3 months.
In diphtheria:

• once daily, for a course of 8-10 suppositories. In cases of diphtheria carrier status - once every 3 days - 3-5 suppositories.
In the treatment of burns of III-IV degree with signs of toxemia, septic toxemia, in surgical patients with septic endocarditis, long-term non-healing wounds of the extremities, purulent-septic complications:

• once daily, for a course of 7-10 suppositories; if necessary, the course of the drug should be continued up to 20 suppositories.
In bronchial obstruction syndrome, cholecystopancreatitis, rheumatoid arthritis:

• once every 3 days, for a course of 8-10 suppositories; if necessary, the course should be continued up to 20 suppositories with the same interval.
In the treatment of psoriasis:

• once daily, for a course of 15-20 suppositories.
In adults as part of vaccination prophylaxis, once on the day of vaccination.

При лечении онкологических больных в схеме радикального комбинированного лечения (химиолучевая терапия и операция):

• однократно, ежедневно, курс лечения 8-10 суппозиториев перед химиолучевой терапией и операцией с последующим продолжением курса в течение всего периода лечения.
У больных с распространенным опухолевым процессом (III - IV стадии) различной локализации в виде комплексной или симптоматической терапии:

• однократно, ежедневно, курс лечения 8 - 10 суппозиториев, с перерывом 15-20 дней и повторением курсов в течение всего периода последующего лечения.
У детей со злокачественными заболеваниями кроветворной и лимфоидной ткани:

• 1 раз в сутки, ежедневно, курс лечения 10-20 суппозиториев. Назначение препарата следует проводить в течение всего курса химиолучевой терапии и после окончания курса для профилактики развития токсикоза.
В комплексной терапии детей с паппиломатозом гортани и ротоглотки:

• 1 раз в сутки, ежедневно, курс лечения 10 суппозиториев.
При оппортунистических инфекциях (цитомегаловирусная и герпетическая инфекция, токсоплазмоз, хламидиоз, пневмоцистоз, криптоспоридиоз):

• 1 раз в сутки, ежедневно, курс лечения 15-20 суппозиториев.
В комплексной терапии ВИЧ-инфекции:

• 1 раз в сутки, ежедневно, курс лечения 15-20 суппозиториев. При необходимости следует проводить повторные курсы через 2-4 недели.
При хроническом вирусном гепатите и хроническом бруцеллезе:

• 1 раз в сутки, ежедневно, курс лечения 15-20 суппозиториев, для предотвращения рецидива следует проводить повторные курсы через 2-3 месяца.
При дифтерии:

• 1 раз в сутки, ежедневно, курс лечения 8-10 суппозиториев. При дифтерийном бактерионосительстве -1 раз через 3 суток - 3-5 суппозиториев.
При лечении ожогов III-IV степени с явлениями токсемии, септикотоксемии, у хирургических больных с септическим эндокардитом, длительно незаживающими ранами конечностей, гнойносептическими осложнениями:

• 1 раз в сутки, ежедневно, курс лечения 7-10 суппозиториев, при необходимости курс препарата следует продолжать до 20 суппозиториев.
При бронхо-обструктивном синдроме, холецистопанкреатите, ревматоидном артрите:

• 1 раз через 3 суток, курс лечения 8-10 суппозиториев, в случае необходимости курс следует продолжать до 20 суппозиториев с тем же интервалом.
При лечении псориаза:
-1 раз в сутки, ежедневно, курс лечения 15-20 суппозиториев.
У взрослых в схеме вакцинопрофилактики однократно в день вакцинации.

Composition

Arginyl-alpha-aspartyl-lysyl-valyl-tyrosyl-arginine diacetate (calculated as anhydrous and free from acetic acid substance) 90 mcg
Excipients: glycine (5 mg), polysorbate 80 (1% of the mass of the suppository), purified water (25 mg), solid fat (to obtain a suppository mass of 1.2 to 1.5 g)

Homogeneous suppositories from white to white with a yellowish tint in color, torpedo-shaped. A faint characteristic odor is permissible.

Pharmacology

The drug has immunoregulatory, detoxifying, and hepatoprotective effects and causes the inactivation of free radical and peroxide compounds. The pharmacological action is based on achieving three main effects: correction of the immune system; restoration of the balance of oxidative-antioxidative reactions in the body; and inhibition of multidrug resistance mediated by transmembrane transport proteins of the cell.

The action of the drug begins to develop within 2-3 hours (rapid phase) and lasts up to 4 months (intermediate and slow phases).
During the rapid phase (duration - up to 2-3 days), the detoxification effect is primarily manifested - antioxidant protection of the body is enhanced by stimulating the production of ceruloplasmin, lactoferrin, and catalase activity; the drug normalizes lipid peroxidation, inhibits the breakdown of cell membrane phospholipids, and the synthesis of arachidonic acid, subsequently reducing blood cholesterol levels and the production of inflammatory mediators. In cases of toxic and infectious liver damage, the drug prevents cytolysis, reduces transaminase activity, and lowers bilirubin levels in the blood serum.
During the intermediate phase (begins after 2-3 days, duration - up to 7-10 days), there is an enhancement of phagocytosis reactions and the destruction of intracellular bacteria and viruses.
During the slow phase (begins to develop on days 7-10, duration up to 4 months), the immunoregulatory action of Immunofan is manifested - restoration of disrupted cellular and humoral immunity parameters. During this period, normalization of the immunoregulatory index is observed, along with an increase in the production of specific antibodies. The effect of Immunofan on the production of specific antiviral and antibacterial antibodies is equivalent to the action of certain therapeutic vaccines. Unlike the latter, the drug does not significantly affect the production of IgE class reagin antibodies and does not enhance immediate-type hypersensitivity reactions; Immunofan stimulates the formation of IgA in cases of its congenital deficiency.
Immunofan effectively suppresses multiple drug resistance of tumor cells and increases their sensitivity to the action of cytostatic drugs.

Препарат обладает иммунорегулирующим, детоксикационным, гепатопротективным действием и вызывает инактивацию
свободнорадикальных и перекисных соединений. Фармакологическое действие основано на достижении трех основных эффектов: коррекция иммунной системы, восстановление баланса окислительно-антиокислительной реакции организма и ингибирование множественной лекарственной устойчивости, опосредованной белками трансмембранного транспортного насоса клетки.

Действие препарата начинает развиваться в течение 2-3 часов (быстрая фаза) и продолжается до 4 месяцев (средняя и медленная фазы).
В течение быстрой фазы (продолжительность - до 2 - 3 суток) проявляется прежде всего детоксикационный эффект - усиливается антиоксидантная защита организма путем стимуляции продукции церулоплазмина, лактоферрина, активности каталазы; препарат нормализует перекисное окисление липидов, ингибирует распад фосфолипидов клеточной мембраны и синтез арахидоновой кислоты с последующим снижением уровня холестерина крови и продукции медиаторов воспаления. При токсическом и инфекционном поражении печени препарат предотвращает цитолиз, снижает активность трансаминаз и уровень билирубина в сыворотке крови.
В течение средней фазы (начинается через 2-3 суток, продолжительность - до 7 - 10 суток) происходит усиление реакций фагоцитоза и гибели внутриклеточных бактерий и вирусов.
В течение медленной фазы (начинает развиваться на 7 - 10 сутки, продолжительность до 4 месяцев) проявляется иммунорегуляторное действие Имунофана - восстановление нарушенных показателей клеточного и гуморального иммунитета. В этот период наблюдается нормализация иммунорегуляторного индекса, отмечается увеличение продукции специфических антител. Влияние Имунофана на продукцию специфических противовирусных и антибактериальных антител эквивалентно действию некоторых лечебных вакцин. В отличие от последних препарат не оказывает существенного влияния на продукцию реагиновых антител класса IgE и не усиливает реакцию гиперчувствительности немедленного типа; Имунофан стимулирует образование IgA при его врожденной недостаточности.
Имунофан эффективно подавляет множественную лекарственную устойчивость опухолевых клеток и повышает их чувствительность к действию цитостатических препаратов.
